Provincial Conference: Innovative Primary Care Models for Seniors’ Health
Be one of the 150 primary health care practitioners (physicians, nurses, allied health professionals), researchers, and representatives from community partner organization at this Provincial conference. Heighten your awareness of frailty, its impact on the health care system, and the essential role of primary care. Learn about current innovative models of seniors’ care and take home strategies to improve inter-professional communication and practice management. Hear from patients and caregivers on care experiences in the community. Network with other attendees and explore the provincial context of system-wide change being pursued to enhance primary health care.
OBJECTIVES:
✓ Explore the Provincial context for system-wide change to enhance primary health care and current innovative models for seniors being implemented within primary care in Alberta; ✓ Discuss inter-professional and inter-organizational communication as contributors to successful integration of innovative programs and initiatives;
✓ Discuss the impact of frailty as a key driver of current health care system costs and explore the foundational role of primary health care in supporting seniors living with frailty;
✓ Recognize patient and family/caregiver experiences with the current healthcare system.
